Zanidatamab with and without Tislelizumab in HER2-Positive Gastroesophageal Cancer

Nel carcinoma gastrico e della giunzione gastroesofagea HER2-positivo, trastuzumab associato alla chemioterapia ha rappresentato per anni il backbone del trattamento in I linea. Più recentemente, l’aggiunta dell’anti-PD-1 Pembrolizumab nei casi con CPS ≥1 ha ulteriormente ampliato le possibilità terapeutiche. Rimane tuttavia il limite dell’eterogeneità dell’espressione di HER2, caratteristica particolarmente rilevante nei tumori gastroesofagei e potenziale causa di resistenza (primaria o acquisita).
Zanidatamab è un anticorpo bispecifico diretto contro due epitopi extracellulari distinti di HER2. Il doppio legame favorisce il clustering e l’internalizzazione del recettore e rappresenta il razionale per un’inibizione più completa del pathway di HER-2 rispetto al trastuzumab.
Lo studio HERIZON-GEA-01 è uno studio internazionale, randomizzato, open-label di fase III che ha arruolato 914 pazienti con adenocarcinoma gastrico, della giunzione gastroesofagea o dell’esofageo avanzato HER2-positivo (confermato mediante revisione centralizzata), non precedentemente trattati per malattia avanzata. L’85% dei pazienti arruolati era HER-2/3+ e il 60% dei pazienti era anche TAP score ≥ 1.
I pazienti sono stati randomizzati in rapporto 1:1:1 a ricevere: zanidatamab + tislelizumab + chemioterapia (n=302), zanidatamab + chemioterapia (n=304), oppure trastuzumab + chemioterapia (n=308). Endpoint primari: PFS e OS.
Ad un follow-up mediano di 25,9 mesi, la PFS mediana è risultata pari a 12,4 mesi in entrambi i bracci contenenti zanidatamab, rispetto agli 8,1 mesi del braccio trastuzumab + chemioterapia . Rispetto al controllo, il rischio di progressione o morte è risultato ridotto del 37% ( HR 0.63; IC95% 0,51–0,78; p<0,001) con zanidatamab + tislelizumab + chemioterapia e del 35% con zanidatamab+ chemioterapia (HR di 0,65 ;IC95% 0,52–0,81 p<0,001).
La mOS è risultata di 26,4 mesi con la tripletta contenente tislelizumab rispetto ai 19,2 mesi con trastuzumab + chemioterapia, con HR di 0,72 (IC95% 0,57–0,90; p=0,004). Nel braccio zanidatamab + chemioterapia la mOS è stata 24,4 mesi con HR 0,80 (IC95% 0,64–1,01; p=0,06), senza raggiungere la significatività statistica prevista all’analisi intermedia.
L’incidenza di eventi avversi di grado ≥ 3 è stata par all’83,3% nel braccio con zanidatamab + tislelizumab + chemioterapia, del 73,8% dei pazienti trattati con zanidatamab + chemioterapia e del 74,5% nei pazienti che hanno ricevuto trastuzumab + chemioterapia. La diarrea è stata l’evento avverso di grado ≥ 3 più frequente, osservato rispettivamente nel 24,8%, 20,0% e nel 12,9% dei casi.
In conclusione, lo studio ha dimostrato un vantaggio in termini di PFS con l’aggiunta dello zanidatamab, in associazione o meno a tislelizumab, alla chemioterapia, rispetto alla combinazione di trastuzumab + chemioterapia. Diversamente, il beneficio in sopravvivenza globale è stato dimostrato, alla presente analisi ad interim, esclusivamente con la tripletta zanidatamab + tislelizumab + chemioterapia (OS mediana 26,4 vs 19,2 mesi; HR 0,72; IC95% 0,57–0,90; p=0,004). L’associazione zanidatamab + chemioterapia, pur mostrando infatti una OS mediana numericamente superiore rispetto al controllo (24,4 vs 19,2 mesi), non ha determinato un miglioramento statisticamente significativo della OS (HR 0,80; IC95% 0,64–1,01; p=0,06). È tuttavia importante sottolineare che il trial non è stato disegnato per un confronto diretto tra i due bracci sperimentali e che il braccio di controllo non includeva un anti-PD-1, come nell’attuale pratica clinica. Il dato più solido è che il targeting bispecifico di HER2 consente sicuramente un controllo di malattia più prolungato rispetto a trastuzumab nella malattia HER-2 positiva. La maturazione dei dati di OS e le analisi dei sottogruppi biomolecolari saranno sicuramente rilevanti per definire come integrare zanidatamab negli algoritmi terapeutici nella prima linea della malattia HER2-positiva.

La chirurgia rappresenta ad oggi l’unica opzione potenzialmente curativa per il colangiocarcinoma intraepatico resecabile sebbene il rischio di recidiva rimanga elevato anche dopo resezioni R0. Tuttavia, le evidenze a favore di una strategia neoadiuvante sono state finora limitate in questo subset di pazienti.
Lo studio clinico ZSAB-neoGOLP è un trial multicentrico, open-label, randomizzato di fase II–III condotto in 11 ospedali cinesi. Sono stati arruolati 178 pazienti con colangiocarcinoma intraepatico resecabile e almeno una caratteristica di alto rischio, definita come diametro tumorale >5 cm, invasione vascolare, malattia multifocale, metastasi ai linfonodi ilari epatici o valori di CA 19.9 > 37 U/mL , randomizzati a trattamento neoadiuvante secondo schema GOLP somministrato ogni 3 settimane per 3 cicli (Gemcitabina 1000 mg/m2 ev d1,8, Oxaliplatino 85 mg/m2 ev d1, Toriparlimab 240 mg ev d1, Lenvatinib 8 mg per os 1 volta/die continuativamente per 9 settimane) seguito da chirurgia (88) oppure a chirurgia upfront (90).
Entrambi i gruppi hanno ricevuto capecitabina adiuvante per otto cicli dopo chirurgia.
Endpoint primario dello studio: event-free survival EFS; endopoint secondari: OS e safety.
Ad un follow-up mediano di 16,9 mesi, la EFS mediana è risultata pari a 18,0 mesi (IC95% 13,8–27,6) nel gruppo neoadiuvante e 8,7 mesi (IC95% 7,2–12,4) nel gruppo di controllo (p<0,001). La probabilità di essere liberi da evento a 24 mesi è stata pari rispettivamente al 37% con GOLP e al 25% con chirurgia upfront.
All’analisi intermedia, la OS a 24 mesi è stata del 79% nel gruppo neoadiuvante e del 61% nel controllo, con HR per morte dello 0,43 (IC95% 0,23–0,79; p=0,005). Il valore di p non ha tuttavia raggiunto la soglia di significatività prespecificata per questa analisi intermedia (alpha bilaterale 0,0019). Il trial è pertanto formalmente positivo per EFS, mentre il dato di OS deve essere considerato ancora immaturo.
Anche i dati preoperatori sono molto interessanti: Il trattamento GOLP ha infatti determinato una ORR del 55%, una major pathological response del 19% e una pCR del 5%. La resezione R0 è stata ottenuta nel 95% dei pazienti del gruppo neoadiuvante e nel 93% del gruppo di controllo, suggerendo come l’introduzione della terapia preoperatoria non abbia compromesso la radicalità chirurgica. Durante la fase neoadiuvante, eventi avversi di grado ≥ 3 sono stati osservati nel 28% dei pazienti e quelli correlati al trattamento nel 26%; non sono stati riportati decessi correlati alla terapia.
Il trial ZSAB-neoGOLP fornisce la prima evidenza randomizzata di un vantaggio clinicamente rilevante di una strategia neoadiuvante in questo setting. Restano tuttavia alcuni limiti. Lo studio è stato condotto esclusivamente in Cina e in centri ad alto volume. Inoltre, il regime combina quattro farmaci e non consente di definire il contributo relativo dei singoli componenti. Anche la selezione dei pazienti è un elemento centrale: il beneficio si è infatti osservato in una popolazione con caratteristiche di alto rischio e non può essere automaticamente esteso a tutti gli iCCA resecabili.
Nel complesso, il vantaggio in termini di EFS supporta l’ipotesi per la quale nei pazienti affetti da colangiocarcinoma intraepatico resecabile ad alto rischio il trattamento sistemico possa essere anticipato rispetto alla chirurgia. La maturazione dei dati di OS e la validazione in popolazioni geograficamente differenti saranno necessarie per stabilire se questa strategia potrà modificare l’attuale standard terapeutico.

La trombocitopenia indotta da chemioterapia (CIT) può compromettere la dose density e la dose intensity dei trattamenti oncologici. A differenza della neutropenia, per la CIT non erano finora disponibili terapie accessibili e approvate, né evidenze di fase III a supporto di un trattamento in grado di prevenire ritardi, riduzioni di dose, omissioni o interruzioni della chemioterapia conseguenti alla trombocitopenia
Romiplostim è un agonista del recettore della trombopoietina (TPO-RA), in grado di stimolare la megacariopoiesi e di incrementare la produzione piastrinica. Studi prospettici precedenti avevano già fornito un segnale consistente di attività di questa molecola nella CIT, in assenza di dati di efficacia in trial randomizzati di fase III. Il trial RECITE è uno studio internazionale di fase III, randomizzato, in doppio cieco e controllato con placebo condotto in pazienti affetti da neoplasie gastrointestinali (75% carcinoma colorettale, il 13% neoplasia gastroesofagea e il 12% carcinoma pancreatico) in trattamento polichemioterapico contenente oxaliplatino e CIT persistente. Sono stati inclusi complessivamente 165 pazienti randomizzati in rapporto 2:1 a trattamento con romiplostim (109) vs placebo (56). Il farmaco è stato somministrato nei primi 3 cicli di chemioterapia.
Endpoint primario dello studio: assenza, sia al secondo sia al terzo ciclo di chemioterapia, di modifiche della dose dei farmaci chemioterapici dovute alla CIT, quali riduzioni, ritardi, omissioni o interruzioni. L’endpoint è stato raggiunto nell’84% dei pazienti trattati con romiplostim (92/109) e nel 36% di quelli trattati con placebo (20/56), con odds ratio pari a 10,16 (IC95% 4,44–23,72; p<0,001) e risk ratio pari a 2,77 (IC95% 1,78–4,30; p<0,001).
Il profilo di sicurezza di romiplostim è risultato complessivamente gestibile. Gli eventi avversi giudicati dagli sperimentatori correlati al trattamento in studio sono stati riportati nel 12% dei pazienti trattati con romiplostim e nel 7% di quelli trattati con placebo; i più frequenti sono stati nausea ( 2% in entrambi i gruppi) e cefalea (2 % nel gruppo romiplostim). Eventi tromboembolici sono stati osservati nel 2% dei pazienti trattati con romiplostim e in nessun paziente del gruppo placebo.
Il Trial RECITE rappresenta un avanzamento rilevante nella gestione della trombocitopenia indotta da chemioterapia, un ambito nel quale la pratica clinica si è finora basata prevalentemente sulla modifica della posologia dei farmaci chemioterapici. La forza dello studio risiede non soltanto nel disegno randomizzato, in doppio cieco e controllato con placebo, ma anche nella scelta di un endpoint direttamente correlato alla pratica oncologica.
L’entità dell’effetto è rilevante: l’84% dei pazienti trattati con romiplostim, rispetto al 36% con placebo, ha proseguito il secondo e il terzo ciclo senza modifiche CIT-correlate. Il risultato dimostra quindi che la stimolazione della trombopoiesi può tradursi in un beneficio operativo concreto, andando oltre il semplice incremento della conta piastrinica.
È tuttavia essenziale non attribuire al trial conclusioni che il suo disegno non consente. Il trial RECITE non dimostra che il mantenimento della dose intensity ottenuto con romiplostim determini un miglioramento di PFS o OS. Il beneficio documentato riguarda la continuità del trattamento chemioterapico e il suo valore clinico dipenderà pertanto dal setting di malattia e dall’importanza attribuibile alla dose intensity nel singolo paziente.
Un ulteriore elemento da considerare è il rischio tromboembolico. Sebbene gli eventi siano stati numericamente limitati (2% con romiplostim e 0% con placebo), il dato assume particolare rilevanza in una popolazione oncologica già predisposta alla trombosi e suggerisce la necessità di un’attenta selezione dei pazienti.
Infine, la popolazione del trial RECITE è stata limitata a neoplasie gastrointestinali trattate con schemi polichemioterapici contenenti oxaliplatino e con CIT persistente: la generalizzazione dei risultati ad altri contesti non è pertanto supportata direttamente dallo studio. Più che introdurre un trattamento da applicare indiscriminatamente alla CIT, lo studio offre pertanto una nuova opzione terapeutica per quei pazienti nei quali preservare la continuità e l’intensità del trattamento rappresenta un obiettivo clinicamente rilevante.
