HER2CLIMB-05: A Phase 3 Study of Tucatinib Versus Placebo in Combination with Trastuzumab and Pertuzumab as First-line Maintenance Therapy for HER2+ Metastatic Breast Cancer

Al SABCS 2025 sono stati presentati i risultati aggiornati dello studio HER2CLIMB-05. Si tratta di uno studio di fase III che prevede l’aggiunta di tucatinib a trastuzumab e pertuzumab rispetto a standard of care (Trastuzumab e Pertuzumab) come terapia di mantenimento di I linea per neoplasie della mammella nel setting avanzato HER2+. L’endpoint primario è stata la PFS con un beneficio di 8.6 mesi dall’aggiunta di tucatinib alla terapia di mantenimento standard con trastuzumab e pertuzumab, dopo terapia di induzione con taxani e doppio blocco anti-HER2, e in associazione a endocrinoterapia per le pazienti che esprimono anche recettori ormonali. Il beneficio è maggiormente presente nel sottogruppo HR- (oltre i 12 mesi) rispetto al gruppo HR+ (6.9 mesi). Per quanto attiene il profilo di tollerabilità, come atteso c’è stato un incremento della diarrea e degli indici di funzionalità epatica nel braccio delle pazienti trattate con tucatinib. Il tasso di discontinuazione di tucatinib è stato del 13.5%, in particolare a causa di ipertransaminasemia. Tale tossicità è tuttavia reversibile dopo opportune modifiche terapeutiche. Si tratta in definitiva di uno schema di mantenimento chemo-free che consente di migliorare nettamente la PFS di queste pazienti e rappresenterà probabilmente un nuovo paradigma nella pratica clinica.



The DEBBRAH trial: Trastuzumab deruxtecan in HER2-positive and HER2-low breast cancer patients with leptomeningeal carcinomatosis

Background: Leptomeningeal disease (LMD) is associated with poor survival and diminished quality of life. Si tratta di uno studio di fase II di grande interesse, a singolo braccio, open-label, con 5 coorti, che ha incluso una popolazione di pazienti affetti da neoplasia mammaria HER2 positiva e HER2-low con coinvolgimento intracranico e leptomenigeo. Focalizziamo l’attenzione sui risultati della coorte 5 che comprende nello specifico sette pazienti con interessamento leptomenigeo. I pazienti ricevevano trastuzumab deruxtecan al dosaggio standard di 5.4 mg/kg ev ogni 21 giorni fino a progressione di malattia o tossicità inaccettabile. L’endpoint primario era l’OS. I secondary endpoint erano la PFS e la safety. La mediana di OS raggiunta è stata di 13.3 mesi, la mediana di PFS è stata di 8.9 mesi. Due dei 7 pazienti sono stati trattati per 18.6 e 11.9 mesi rispettivamente. Dei restanti 5 pazienti andati incontro a progressione e morte, nessuno ha manifestato progressione di malattia a livello intracranico o peggioramento dei sintomi leptomeningei, a fronte di una safety mantenuta. Il trastuzumab deruxtecan ha mostrato, pertanto, significativa attività intracranica ed extracranica e rappresenta una importante chance terapeutica in un gruppo di pazienti a prognosi infausta.

 


Associations of the HER2DX Genomic Test with Biological and Pathological Features in HER2-positive Breast Cancer

HER2DX è un test genomico validato utilizzato per supportare le decisioni terapeutiche nel carcinoma mammario HER2-positivo (HER2+) in stadio iniziale. Fornisce tre punteggi: rischio di recidiva, probabilità di risposta patologica completa (pCR) ed espressione dell’mRNA di ERBB2. Questo studio mirava a valutare l’associazione tra HER2DX e caratteristiche istopatologiche e a valutarne la relazione con la pCR dopo terapia neoadiuvante. Le pazienti con carcinoma mammario HER2+ di nuova diagnosi in stadio I-III sono state analizzate sulla base dei risultati HER2DX disponibili durante i trattamenti standard presso centri oncologici in Spagna (gennaio 2022-giugno 2025). Il test HER2DX centralizzato è stato eseguito su campioni tumorali fissati in formalina e inclusi in paraffina. L’analisi istopatologica ha incluso il grado tumorale, lo stato del recettore ormonale (HR), il sottotipo istologico, l’indice Ki67, il punteggio immunoistochimico (IHC) di HER2, i linfociti infiltranti il tumore stromale (TIL), le strutture linfoidi terziarie (TLS) e la distribuzione immunitaria spaziale. Sono state condotte analisi di regressione univariata e multivariata per identificare i fattori associati alla pCR dopo terapia neoadiuvante a base di trastuzumab. Sono stati analizzati 410 tumori HER2+ e 250 pazienti hanno ricevuto una terapia neoadiuvante a base di trastuzumab (il 36,0% ha ottenuto una pCR). I punteggi pCR HER2DX erano significativamente associati a tutte e otto le caratteristiche istopatologiche. Possiamo concludere che HER2DX riflette le principali caratteristiche biologiche e patologiche del carcinoma mammario HER2+ e predice in modo indipendente la pCR, supportando la sua utilità per il processo decisionale terapeutico individualizzato.

FDA Approves Subcutaneous Amivantamab for EGFR+ NSCLC

Dec 18, 2025 – The subcutaneous formulation of amivantamab and hyaluronidase is noninferior to the IV version, with similar pharmacokinetics and efficacy in NSCLC with EGFR mutations.
Subcutaneous administration showed longer median overall survival and reduced infusion-related reactions and venous thromboembolism rates compared to IV administration.
Patients reported higher convenience with the subcutaneous formulation, which also had significantly shorter administration times .

FDA Grants Full Approval to Rucaparib for BRCA Mutation–Associated mCRPC

Dec 17, 2025 – Rucaparib is now FDA-approved for BRCA mutation-associated mCRPC after prior androgen receptor-directed therapy, based on phase 3 TRITON3 trial results.
The trial showed rucaparib significantly improved radiographic progression-free survival compared to physician’s choice, with medians of 11.2 vs 6.4 months. Median overall survival was 23.2 months with rucaparib versus 21.2 months with physician’s choice, indicating a slight advantage. .
