Comparison of PARPis with Angiogenesis Inhibitors and Chemotherapy for Maintenance in Ovarian Cancer: A Network Meta-Analysis
Seventy-five percent of ovarian cancer would relapse within 18-28 months after platinum-base chemotherapy. Evidence suggests that maintenance chemotherapy is effective in prolonging remission. Recent target therapies such as poly(ADP-ribose) polymerase inhibitors (PARPis) and angiogenesis inhibitors (AIs) are known to ease burden and recurrence of ovarian cancer. La revisione della letteratura ha coinvolto 24 trial clinici e 11.300 pazienti con carcinoma ovarico sottoposte a terapia di mantenimento con PARP inibitori, antiangiogenici e chemioterapia, e ha riportato che il mantenimento con PARP inibitori e antiangiogenici si associa ad un significativo aumento della sopravvivenza libera da progressione (PFS) rispetto al placebo, mentre lo stesso vantaggio non è descritto quando il mantenimento si fa con la stessa o una diversa chemioterapia. Inoltre il vantaggio dei PARP inibitori in termini di PFS è superiore a quello ottenuto con gli antiangiogenici, ma questo dato va interpretato con cautela perché i PARP inibitori di solito vengono somministrati al termine della chemioterapia nelle pazienti platino responsive e quindi a migliore prognosi, mentre gli antiangiogenici sin dall’inizio della chemioterapia a una popolazione non selezionata per la risposta. In termini di eventi avversi, una riduzione del numero di pazienti che discontinuano il trattamento per tossicità viene riportata nei trial clinici che hanno usato PARP inibitori rispetto a quelli che hanno usato antiangiogenici, ma questa differenza non è significativa.
