Cari colleghi, il crescere di interesse e di risultati nel campo dell¹immunoterapia su svariate patologie oncologiche, incluse quelle del distretto testa-collo, fanno sì che ci sia sempre più necessità di tenersi aggiornati ed informati. Impresa sicuramente non semplice se si pensa che i lavori preclinici e clinici in altri settori, pubblicati da gennaio ad oggi sulle riviste indicizzate, sono stati moltissimi anche se quelli specifici sulle neoplasie di testa e collo sono stati solo 7. Con questo nuovo strumento AIOM si prefigge unicamente di segnalare alcuni dei principali paper pubblicati nel settore immunologico e con particolare riferimento all'oncologia del distretto testa-collo, senza voler enfatizzare alcuni argomenti più che altri, ma con il solo obiettivo di facilitare una selezione da parte dei soci in un momento di cosi elevato interesse scientifico sull'argomento. |

| The tumor microenvironment of squamous cell carcinoma of the head and neck (SCCHN) has been shown to be immune suppressive. Therefore, strategies aimed at overcoming this issue could have a positive therapeutic impact. | Montler et al. hanno studiato alcuni recettori presenti sui TIL: OX40, CTLA-4 e PD-1. Sono tre le informazioni utili: 1) il valore dei recettori studiati è maggiore nei TIL piuttosto che nelle corrispettive popolazioni presenti nel sangue periferico degli stessi pazienti. Sarebbe utile anche una valutazione su volontari sani, che ci permetterebbe un confronto e quindi eventualmente di ipotizzare l'uso dei valori ematici come indicatori, nonostante la non corrispondenza con i valori intratumorali. 2) Il valore di OX 40 (CD134), un co-recettore attivatore, ma che sembra avere effetto inibitorio sulle Treg, dove, fra l'altro, nello studio di Montler sembrerebbe più espresso che nelle altre sottopopolazioni di TIL. Inoltre, 3) OX 40, se attivato sui TIL effettori, ha la capacità di inibire la trasformazione degli stessi in Treg, colpendo ulteriormente questa importante popolazione inibitoria. Gli autori concludono che tutti questi recettori sono target interessanti per la terapia. |

| È risaputo che INFy è un potente induttore di PD-L1. Concha-Benavente et al. osservano che INFy e EGFR utilizzano lo stesso nodo di trasmissione del segnale: JAK2. Osservano che overespressione di wtEGFR si correla con JAK2 e PD-L1 overespressione e suggeriscono un ruolo per gli inibitori di JAK2/STAT1 in clinica. |
